Insurance forms for minors to enter the pits can be found on our website, http://www.lincolnparkspeedway.com/g...surance-forms/. Please note that both the 1068 and the 1068 NS must be filled out and printed off in color for pit admission.

Rules
1) The forms must be in color
2) Both parents need to sign on the parent side of the 1068 and 1068NS
3) Birth Certificate is required for all minors.
4) The 1068NS must be notarized. Pit gate attendants can do this for you if you wait to do signatures at the track.
5) If child is unable to write, on the minor side put the child's name followed with "parent's name"

**You may email the child's birth certificate and a current picture to office@spikerpromotions.com and we will have the minor card ready at the pit gate when you turn in forms. Emails must be received by noon the day prior to event, or the card will have to be done onsite.
