If I remember correctly there were 2 issues that have been addressed. 1 the caution lights were never activated and 2 race tower was on a different channel and drivers never heard about the car on the front stretch. Hopefully they have learned from mistakes. They are human and mistakes will always happen.
This was not in the Silver Crown race, but in the Sprint race after. It was not USAC sanction.
