2019 TONY ELLIOTT FOUNDATION GOLF TOURNAMENT

DATE: Thursday, September 5, 2019

TIME: 8:30am Registration. 9:30am Shotgun start

PLACE: Brickyard Crossing, 4400 W. 16th Street, Indianapolis, IN

ENTRY FEE: $125.00 ($500.00 per Foursome) Includes green fees, cart & catered lunch.

Payment due August 31, 2019. Send check to mailing address below or use Paypal on website: www.tonyelliottfoundation.com.

FORMAT: Tournament will be played in a Scramble format with 4-person teams. If you do not enter a team list, you will be placed with others.

AWARDS: Prizes for 1st, 2nd and 3rd place teams.

CANCELLATION POLICY: If golf tournament is canceled due to weather, we thank you for your donation. If player is a “no-show”, entry fee is non-refundable.
